MECHANICS

1. Patients will be entitled to a raffle coupon for every fully paid-taken transaction worth PHP 3,990.00 in a single receipt transaction, outright and concessionary.
a. Example: PHP 15,000.00 worth of purchase single transaction = 3 raffle coupons

2. Promo is available for Cash and Card purchases

3. Customer shall fill-up the raffle coupon with below information and drop in drop boxes located in all George Optical branches:
a. Complete Name
b. Address
c. Contact Numbers/Mobile Number
d. Email Address
e. Affixed Signature

4. Giving of Raffle Coupons will be from purchases made starting February 1, 2010 to March 31, 2010. Deadline of submission of entries will be on March 31, 2010 at 9:00 pm

5. Draw Date is on April 14, 2010 to be held at George Optical Head Office with address:
i. 4th Flr., G&B Bldg., 2005 Taft Ave., near cor. Gil Puyat Ave., Pasay City
ii. To be officiated by Dr. Geoffrey Co (President / CEO), assisted by the Marketing Department and witnessed by a DTI representative.

6. One (1) winner will be draw for trip to boracay for 2 including:
a. Round trip airfare
b. Island transfer
c. Hotel accommodation (3 days 2 nights)

7. Trip prize is transferable but not convertible to cash.

8. Winner should avail of his vacation trip within One (1) year from the date of claim of certificate.

9. Winners will be notified through registered mail and will be posted in all George Optical branches.

10. Redemption period shall be within 60 days from receipt of notification.

11. For GMA and/or Provincial winner, redemption of certificate shall be made at George Optical Head office located at 2005 G&B Bldg. Taft ave. corner Gil Puyat ave., Pasay City.

12. Winner upon claiming the certificate should present the claim stub, valid ID, and registered mail.

13. Employees of George Optical, Inc. and G&B Laboratories and their relatives up to 2nd degree of consanguinity and affinity are disqualified to join the promo
a. Father, mother, father-in-law, mother-in-law, spouse, children, step-children, sisters, brothers, half-sister, half-brother, sister-in-law, brother-in-law, etc.

14. In case the winner fails to claim his certificate 60 days from the date of the receipt of the registered mail, the prize will automatically be forfeited with prior DTI approval.
